چکیده

Liver resection is the definitive treatment for several benign and malignant liver diseases. Experience with laparoscopic procedures and recent advances in laparoscopic devices have created an evolving interest in the application of these techniques to liver resection. However, laparoscopic liver resections may be technically demanding. Pedicle control is an important step of liver resection. Anatomic hepatectomies usually require extensive hilar dissection. To facilitate pedicle control and reduce operating time, we have previously described a technique for laparoscopic right-liver resections by using the intrahepatic Glissonian approach. Laparoscopic right-liver trisectionectomy is a very complex procedure, and, to our knowledge, there is only one technical description, so far, in the English literature. This video demonstrates technical aspects of a totally laparoscopic right trisectionectomy, using the intrahepatic Glissonian approach. Our technique differs from that previously described by intrahepatic pedicle control and total control of venous outflow, which makes the procedure easier and safer for laparoscopy.

M. A. Machado, R. C. Surjan, T. Basseres and F. F. Makdissi University of Sao Paulo, Sao Paulo, Brazil Objective: Minimal access surgery is moving towards reduced size and fewer ports. In the last few years, a novel technique using single-incision laparoscopic approach has been described. Laparoscopic liver resection has been increasingly used in the past decade but there are few reports on sin...

Anatomical basis for the intrahepatic glissonian approach during hepatectomies.

BACKGROUND Anatomical liver resections are based on some basic technical principles such as vascular control, ischemic area delineation to be resected and maximum parenchymal preservation. These aspects are achieved by the intrahepatic glissonian approach, which consists in accessing the pedicles of hepatic segments within the hepatic parenchyma. Small incisions on well-defined anatomical landm...

Intrahepatic Glissonian approach for pure laparoscopic left hemihepatectomy.

BACKGROUND Recent advances in laparoscopic devices and experience with advanced techniques have increased the indications for laparoscopic liver. AIM The aim of this work was to present a video with technical aspects of a pure laparoscopic left hemihepatectomy (segments 2, 3, and 4) by using the intrahepatic Glissonian approach and control of venous outflow without hilar dissection or the Pri...

Intrahepatic glissonian approach for single-port laparoscopic liver resection.

BACKGROUND Minimal access surgery is moving toward reduced size and fewer ports. The aim of this article is to describe our experience with the intrahepatic Glissonian approach for single-port laparoscopic left lateral sectionectomy. SUBJECTS AND METHODS We have performed this procedure on 8 consecutive patients. A transumbilical incision is performed, and a single-incision platform is introd...
